The Data Controller for this site is Lesley Patchick Counselling and I am are registered with the Information Commissioners Office (ICO) No: ZA384687

They also enable me to see information like how many people use the website and what pages they tend to visit.

Analytics: Lesley Patchick Counselling’s website host Wix does collect behaviour patterns of website visitors, but this does not identify individuals. It does show Lesley Patchick Counselling: the number of site visits, the number of specific page views, and the type of device has been used.

The website page for contacting Lesley Patchick does not store information. I will use your personal data of email address/telephone number for correspondence with you regarding an inquiry about the services of Lesley Patchick Counselling. I will retain this information only for the period we are in correspondence, it is then deleted.

Lesley Patchick Counselling does not use the information you supply for marketing purposes and does not share with third parties unless I am required to by law.

Whilst using email, please be aware that there is no guarantee of security due to how your e mail server works, so please consider the level of personal information you share in an email.

General data protection regulation GDPR information for Lesley Patchick Counselling:

This details how Lesley Patchick Counselling collects, stores/processes your personal data.

If you do become a counselling client I will collect: Your name; contact details and emergency contact name and number and this will be used only for contacting, you whilst you are receiving counselling.

Lesley Patchick Counselling does not share your personal data or use it for marketing purposes or sell it to third parties.  

I may need to share your data for these reasons only:

Legitimate interest

  1. There may be the necessity during counselling to break confidentiality for these reasons only: safeguarding; serious harm to yourself or to someone else; acts of terrorism or drug trafficking/money laundering. In these cases, your information may be shared with health professionals & emergency services as appropriate.

  2. Clinical will. In the event of my death or if I become incapacitated, I have appointed my supervisor to manage my cases.


Legal obligation

  1. I may be required to share information in your notes if I am issued with a court order.

  2. I keep anonymous notes from our sessions in line with the requirements of my professional insurance and the limitation act.

Consent

  1. If you ask me to share your notes/information with other medical professionals or your legal representative.

How long does Lesley Patchick Counselling keep your information?

  1. If you choose not to continue with counselling after your initial session, your information will be disposed of after 4 weeks.

  2. If you choose to continue with counselling, your notes are anonymous and kept for 7 years after your last session (as my professional insurances requires). For under 18’s anonymous notes will be kept for 7 years after the age of 18 is reached.​


​All information is stored as confidential data in locked storage, password protected document or encrypted memory stick and destroyed at the end of the data retention period as confidential waste.

Your Rights under GDPR:

  • You have the Right to be informed;

  • The Right to access the information I have about you;

  • The Right to rectify that information;

  • The Right to restrict the processing of notes.
